I was going to just copy/paste whatever the official bio said on the official website, and that sucked.

General rule: If it'll probably grow back eventually, you can probably just go for it.
If it's something permanent, ask ahead of time.
Basically, you don't need to ask before throwing an IC punch and giving Adachi a black eye. You would need to ask if it's gouging out an eye and leaving him blind. The one exception to this is if it's something that'll heal but will take a lot of time, like giving him a broken leg.
I'm fine with the idea of a broken leg, but it would suck if that happens shortly before I run some highly anticipated player plot in which Adachi sprints for five hours. You know?
